Voluntary Medical Male Circumcisions for HIV Prevention — 13 Countries in Eastern and Southern Africa, 2017–2021

HIVを予防するための男性の自発的医学的割礼 ― アフリカ東部および南部の13カ国、2017年～2021年

男性の自発的医学的割礼(VMMC)が女性から男性へのHIV感染リスクを約60％低減させることと関連することが判明した後、2007年に、WHOとJoint United Nations Programme on HIV/AIDS(UNAIDS)によってVMMCが是認された。この是認の結果、アメリカのPresident’s Emergency Plan for AIDS Relief(PEPFAR)は、CDC、アメリカ国防総省、アメリカ国際開発庁などの政府機関と協力し、アフリカ南部および東部の優先国で実施されるVMMCの支援を開始した。CDCは、2010年～2016年に12カ国で5,880,372件、2017～2021年に13カ国で8,497,297件のVMMCを支援した。2017年～2019年ではCDCが支援するVMMC数が毎年増加し、年間平均増加率は13.5％であったが、2020年ではCOVID-19パンデミックが始まり、VMMC数は2019年の2,120,797件から1,447,147件に31.8％減少し、2021年にはさらに減少した。2017年～2019年のVMMCは43.8％が10～14歳、45.7％が15～29歳に実施された。15～29歳に実施されたVMMCの割合は、2020年の61.0％から2021年には86.6％に増加した。大半(74.2％)のVMMC男性が、VMMCの場所でHIV検査サービスを受けたが、この割合は2017年の86.4％から2021年には48.7％に減少した。VMMCの場所でHIV検査を受けた5,595,239名のうち、44,745名(0.8％)が検査結果陽性であった。HIV陽性率は0.4％(2020年)～1.2％(2021年)であった。PEPFAR Monitoring, Evaluation, and Reportingデータベースの2017年～2021年のデータは、最新情報提供、VMMCプログラムの規模拡大に対するCDCの寄与についての説明に使用された。これは、優先国においてAIDS流行を2030年までに終結させるために15～59歳の男性の90％がVMMCサービスにアクセスするという2025年のUNAIDSの目標を達成させるために重要である。
