Possible Undetected Mpox Infection Among Persons Accessing Homeless Services and Staying in Encampments — San Francisco, California, October–November 2022

2022年7年～10月、サンフランシスコ公衆衛生局はホームレス経験者のサル痘(mpox)患者を確認した。mpox有病率および感染経路の調査のため、2022年10月25日～11月3日、CDCのフィールドチームがカリフォルニア州サンフランシスコにてホームレス支援サービスの利用者、またはテント村、シェルター、永久支援住宅に居住する人のうち、mpox症例発生またはリスクのある集団がいる施設16カ所(シェルター7カ所、サービスセンター5カ所、支援住宅2カ所、テント村2カ所)を訪問し、計209名の15分の調査と血液の採取を完了し、血清中抗オルソポックスウイルス値を測定した。平均年齢は46歳(19～83歳)、男性145名、女性61名であり、非ヒスパニック系白人80名(38.3％)、非ヒスパニック系黒人/アフリカ系アメリカ人66名(31.6％)が多く、147名(70.3％)が非ヒスパニック系であった。mpox感染の既往あり/mpoxワクチン接種した2名を除外した207名のうち50名(23.9％)が天然痘ワクチンのみ、22名(11.5％)がmpoxワクチンのみ、10名(4.8％)が両ワクチンを接種しており、117名(56.0％)は両ワクチンとも接種しておらず、8名(3.8％)は接種歴不明であった。天然痘ワクチンのみ接種者のうち、抗オルソポックスウイルスIgGは19名(38.0％)、IgMは1名(3.2％)にて検出された。mpoxワクチンのみを接種者のうち、抗オルソポックスウイルスIgGは12名(54.5％)、IgMは4名(21.1％)に検出された。両ワクチンとも接種していない人のうち、50歳未満の80名中2名(2.5％)に抗オルソポックスウイルスIgGを検出した。また、mpoxワクチンを接種しておらず、IgM 検査をした68名中1名(1.5％)にIgMを検出した。これらの結果から、mpox感染の可能性があるのは3名であることが判明した。この3名には前月に性的接触はなく、トランスジェンダー、ゲイ、バイセクシュアルであったこともなく、調査時に発疹もなかった。3名中2名が洗っていない食器を共有、発疹のある人のそばで過ごす/接触する、喫煙具を共有などしていた。3名のホームレス経験者の検体にて未検出であったmpox感染の可能性が示されたことから、これらの集団が利用しやすいワクチン接種などの予防介入や地域への働きかけを確実にする必要があることが強調された。
