COVID-19の死亡率とワクチン接種率 ― 香港特別行政区、中国、2022年1月6日～2022年3月21日

2022年1月6日に、COVID-19の原因ウイルスであるSARS-CoV-2のオミクロン変異株に起因するCOVID-19症例クラスターが、中国の香港特別行政区(香港)で検出され、地域のCOVID-19症例の第5波となった。第5波は2022年3月4日に人口100万人あたりのCOVID-19症例8,764例とピークに達し、2022年1月6日～3月21日に、合計1,049,959例の症例と5,906例のCOVID-19関連死亡が香港保健局に報告された。この期間中の香港でのCOVID-19による死亡率(人口100万人あたり37.7人)は、COVID-19のパンデミックが始まって以降、世界で最も高かった。2回接種のプライマリーワクチン接種シリーズ[3歳以上に推奨される不活化COVID-19ウイルスワクチンのSinovac-CoronaVac(Sinovac社製)、5歳以上に推奨されるmRNAワクチンのBNT162b2(Pfizer社-BioNTech社製)]について、2021年12月23日時点での香港における年齢別のワクチン接種率、2022年1月6日～3月21日のCOVID-19による死亡率に関する公表されたデータを分析した。香港のワクチン接種対象者において、2021年12月23日までにCOVID-19ワクチンを1回以上接種したのは67％、2回以上接種は64％、ブースター接種は5％であり、60歳以上におけるこれらの割合はそれぞれ52％、49％、7％であった。60歳以上のワクチン接種率は年齢上昇にともない低下し、1回以上接種、2回以上接種、ブースター接種の割合は、70～79歳では各48％、45％、7％、80歳以上ではそれぞれ20％、18％、2％であった。報告された5,906例のCOVID-19による死亡のうち5,655例(96％)が60歳以上で発生し、これらの死亡者のうち3,970例(70％)はワクチン未接種、 1,023例(18％)は1回接種、662例(12％)は2回以上接種であった。COVID-19ワクチンの未接種、1回接種、2回以上接種した60歳以上の人におけるCOVID-19関連死亡率の全体的な割合は、人口100万人あたりそれぞれ10,076人、1,099人、473人であり、ワクチン未接種者のCOVID-19関連死亡リスクは、この年齢層における2～3回ワクチン接種者の21.3倍であった。2022年の香港オミクロン変異株によるCOVID-19アウトブレイク中の全体的に高い死亡率は、60歳以上のワクチン未接種者における死亡によって引き起こされている。年齢別のワクチン接種率のギャップを特定して対処するための取り組みは、特に60歳以上におけるCOVID-19による高い死亡率を防ぐのに役立つ可能性がある。
