Pediatric Emergency Department Visits Associated with Mental Health Conditions Before and During the COVID-19 Pandemic — United States, January 2019–January 2022

COVID-19パンデミック前とパンデミック中における小児のメンタルヘルス系疾患による救急科受診 ― アメリカ、2019年1月～2022年1月

アメリカではCOVID-19パンデミックによる小児メンタルヘルスの悪化から、2021年、小児メンタルヘルスに関する国家非常事態が宣言された。CDCはNational Syndromic Surveillance Programデータを3期間[2020年3月15日～2021年1月2日(2020年)、2021年1月3日～2022年1月1日(2021年)、2022年1月2～29日(2022年1月)]に分け、0～17歳の小児および思春期齢におけるメンタルヘルス系疾患(MHC)全体および特定のMHC[うつ、不安、破壊行動/衝動制御障害、ADHD、心的外傷およびストレス関連障害、双極性障害、摂食障害、チック障害、強迫性障害(OCD)]による救急科(ED)受診数および受診率を0～4歳、5～11歳、12～17歳にわけ、性別ごとに分析した。0～17歳でのMHC全体の週平均受診率は2019年に比べ2020年にて27％減少したが、2021年および2022年1月はそれぞれ8％、5％の減少と横ばいであった。すべての小児/思春期齢におけるMHC受診数は増加しており、年齢およびMHCの種類によるばらつきが認められた。特に12～17歳の女子ではすべての期間において受診数および受診率がすべてのMHCにて最も増加していた。0～4歳ではMHC全体および特定のMHCによる受診数は少なかった。5～11歳女子ではチック障害による受診数が増加し、2019年に比べた受診率比(VR)は2020年3.04、2021年2.03、2022年1月2.16であった。この年齢層の男子および全体ではこのような変化は認めなかった。12～17歳女子では2019年に比べ2020年は摂食障害、チック障害、2021年はうつ、摂食障害、チック障害およびOCD、2022年1月は不安、心的外傷およびストレス関連障害、摂食障害、チック障害、OCDによる受診およびMHC全体の受診数が増加し、2020年、2021年、2022年1月のVRは、摂食障害でそれぞれ1.95、2.29、1.99、チック障害でそれぞれ3.65、3.94、2.96であった。小児および青年のメンタルヘルスの改善には早期発見とエビデンスに基づく予防的介入戦略が重要であり、特に思春期齢の女子においてニーズが高いことが示唆された。
