CDC Deployments to State, Tribal, Local, and Territorial Health Departments for COVID-19 Emergency Public Health Response — United States, January 21–July 25, 2020

COVID-19の緊急公衆衛生対応のための州、部族自治区、地方、海外領土の保健部門へのCDCの配置 ― アメリカ、2020年1月21日～7月25日

新型コロナウイルス感染症(COVID-19)は、SARS-CoV-2が原因のウイルス性呼吸器疾患である。 2020年1月21日～7月25日、COVID-19の緊急公衆衛生対応活動への支援を必要とする公式の要請に対応して、CDCは55の州、部族自治区、地方、海外領土の保健部門を支援するために208チームを配置した。CDCの配置データを分析し、SARS-CoV-2の伝染を抑制するための対策を確認し、実施するため、州、部族、地方、準州の保健局への支援として配置したCDCチームによる活動をまとめた。配置されたチームは、長期療養施設(53チーム、全体の26％)、食品加工施設(24チーム、12％)、刑務所(12チーム、6％)のようなリスクの高い集団施設と、ホームレス経験者に対する公共施設(10チーム、5％)における感染の調査を支援した。配置された208チームのうち、178チーム(85％)が州、12チーム(6％)が部族自治区、10チーム(5％)が地方、8チーム(4％)が海外領土のそれぞれの保健部門を支援した。保健部門とのCDCの協力により、地域の能力が強化され、アウトブレイクに対応した支援が提供された。この協力体制では、不均衡に影響を受けた集団(人種/民族的マイノリティ集団、最前線で働くエッセンシャルワーカー、ホームレスを経験者など)の健康の公平性の問題や場所に基づいた視点(農村または辺境地域に住む人々)を通して、重点的に取り組んだ。全体で、スタッフの516名(71％)が1回、156名(21％)が2回、54名(8％)が3回以上配置された。1,009名の配置のうち、主要な役割は、疫学的サポート(422名、42％）、指導(137名、14％)、感染防止対策(88名、9％）、臨床サポート(65名、6％)であった。また、これらの協力体制は、COVID-19疫学の特性評価を促進し、高リスクの集団環境での封じ込め戦略としての連続検査のガイダンス、食品加工施設の従業者における対象を絞った介入と予防の取り組み、ソーシャルディスタンスを含むCDCのデータ情報に基づくガイダンスに直接寄与した。
