妊娠中のマリファナ使用の特徴 ― 8州、Pregnancy Risk Assessment Monitoring System、2017年

マリファナはアメリカでもっとも多く使用されている連邦法での違法物質であるが、医療用および成人の非医療用として合法としている州も多い。妊娠中のマリファナ使用は低体重出生など胎児に有害な影響を与える。妊娠中にマリファナを使用する妊婦の人数を推定し、使用方法および使用理由を特定し、妊娠中に使用を続ける女性と使用を止める女性の特徴について理解するため、CDCは、2017年のPregnancy Risk Assessment Monitoring System(PRAMS)に参加する8州(アラスカ州、イリノイ州、メイン州、ニューメキシコ州、ニューヨーク州、ノースダコタ州、ペンシルベニア州、ウエストバージニア州)のデータを分析した。妊娠前、妊娠中および妊娠後のマリファナ使用に関する情報は計6,236名より得られ、自己報告によるマリファナの使用率は妊娠前9.8％、妊娠中4.2％、妊娠後5.5％であった。使用率は州により差があり、妊娠前は8.4％(ニューヨーク州)～21.2％(メイン州)、妊娠中は2.6％(ニューヨーク州)～12.1％(メイン州)、妊娠後は4.4％(イリノイ州)～15.9％(メイン州)であった。妊娠中にマリファナを使用した413名における使用理由は、ストレス/不安の緩和(81.5％)、悪心/嘔吐の緩和(77.8％)、疼痛緩和(55.1％)が多く、他には、楽しくなるため/リラックスするため(45.7％)、慢性症状緩和 (24.9％)などであった。妊娠中におけるマリファナの使用法は吸煙(91.0％)がもっとも多く、他には食べる(12.1％)、蒸気を吸う(7.1％)、塗布(4.5％)、飲む(0.5％)、その他(0.5％)であった。妊娠前および妊娠中にマリファナを使用した765名のうち、41.2％は使用を継続し、58.8％は妊娠中に使用を中止していた。多変量解析による使用を継続した女性の比率は、非ヒスパニック系白人やその他の人種が非ヒスパニック系黒人に比べ、独身女性が既婚女性に比べ、学歴が12年または12年未満の女性が12年より多い女性に比べ、妊娠中の喫煙女性が非喫煙者に比べ、有意に高かった。
