Human Papillomavirus Vaccination for Adults: Updated Recommendations of the Advisory Committee on Immunization Practices

成人に対するヒトパピローマウイルスのワクチン接種：最新のAdvisory Committee on Immunization Practicesの推奨

新たなヒトパピローマウイルス(HPV)感染、がんも含むHPV関連疾患を予防するため、HPVワクチン接種が推奨されている。Advisory Committee on Immunization Practices(ACIP)は、11歳または12歳での定期的なHPVワクチン接種を推奨しており、ワクチン接種は9歳から開始できる。HPVワクチンのキャッチアップ接種は、26歳までの女性に対しては2006年から、21歳までの男性と26歳までの特定の集団に対しては2011年から推奨されている。今回は、2014年、2015年、2016年に発表されたACIPのHPVワクチンのキャッチアップ接種の推奨とガイダンスの更新について報告する。青少年期に対するワクチン接種に関する推奨に変更はない。2018年4月から2019年6月までに、ACIPのHPVワクチンワークグループはEvidence to Recommendations frameworkを使用し、成人のHPVワクチン接種に関する関連する科学的エビデンスを検証した。ワークグループは、HPV感染およびHPV関連疾患の一次予防のためのHPVワクチン接種の有効性、安全性などに関するエビデンスの質を、Grading of Recommendations Assessment, Development and Evaluation(GRADE)アプローチを使用して評価した。27～45歳の成人における9価、4価、2価のHPVワクチン(各9vHPV、4vHPV、2vHP)の臨床試験11件のデータを検討したところ、3つの試験のper-protocol分析では、4vHPVおよび2vHPVは、持続性のワクチン型HPV感染、肛門性器疣贅、子宮頸部上皮内腫瘍(CIN)グレード1(低グレード病変)以上の複合エンドポイントに対して有意な有効性を示した。HPVワクチンを3回投与した後のワクチン型HPVへのセロコンバージョン率を検討した9件の試験では、初回投与後7カ月で93.6％～100％であった。中程度の品質のエビデンスについて、全体的な有益性に関するエビデンスは、GRADEエビデンスレベル2であった。9件の試験では、重篤な有害事象はほとんどなく、ワクチン関連の死亡もなかった。また、HPVによる疾患の負担およびワクチン接種プログラムの影響の調査と医療経済分析を実施した。エビデンスから、HPVワクチン接種は27～45歳の成人にとって安全であるが、公衆衛生の利益は最小限になることが示された。2019年6月に、ACIPはすべての人に26歳までにHPVワクチンのキャッチアップ接種を推奨した。ACIPは、27歳から45歳までの成人に対してはキャッチアップ接種を推奨しなかったが、適切にワクチン接種されておらず、新たなHPV感染リスクがあるかもしれない人では、この年齢範囲でのワクチン接種の恩恵を受ける可能性がある。そのため、ACIPは、これらの人々に対するHPVワクチン接種の可能性に関する臨床的な意思決定を伝えることを推奨した。
