Updated Framework for Development of Evidence-Based Recommendations by the Advisory Committee on Immunization Practices

Advisory Committee on Immunization Practicesによるエビデンスに基づいた勧告策定のための最新のフレームワーク

Advisory Committee on Immunization Practices(ACIP)は、アメリカの一般市民におけるワクチン予防可能疾患のコントロールのためのワクチンおよび関連する薬剤の使用に関する勧告の形で、CDC長官およびアメリカ保健社会福祉省長官に専門家のアドバイスを提供する連邦諮問委員会である。科学情報の収集、分析、作成を行う作業部会は、勧告形成プロセスを支援し、評価した科学的エビデンスに基づいて勧告のための選択肢を提供する。ACIPの投票権を持つ委員の過半数により承認された勧告は、その後CDC長官によって審査され、長官の承認を得てMMWRに掲載される。今回は、ACIPが2018年2月の会議で採択したエビデンスに基づく勧告を策定するためのACIPプロセスの更新をまとめた。2010年に、ACIPは、エビデンスに基づく勧告を作成するために、Grading of Recommendations Assessment, Development and Evaluation(GRADE)によるアプローチを正式に採択した。それ以降、利益と有害に関連する転帰のエビデンスの質の評価を伝えるGRADEエビデンスプロファイルの作成と提示は、ACIP勧告策定の不可欠な部分となっている。一方、勧告の策定過程ではエビデンスの確実性に加え、パネラーは転帰に関する対象集団での価値、保健経済データ、実施問題などの無数の要素を考慮する。この分野の重要な追加は、過去8年間にGRADEに組み込まれてきた。特に、Evidence to Decision or Evidence to Recommendation(EtR)の使用は、勧告を検討する時のエビデンスから決定への移行過程を支援し、決定に対する追加因子の影響の透明性を提供している。ACIPは、2018年2月の会議でEtRフレームワークの使用を採択した。EtRフレームワークは、ワクチンの勧告審議において考慮される追加の要素を明らかにし、医療機関、パートナー機関、および一般の人々への勧告の透明性、一貫性、伝達を促進する。
