Increases in Hepatitis C Virus Infection Related to Injection Drug Use Among Persons Aged ≤30 Years — Kentucky, Tennessee, Virginia, and West Virginia, 2006–2012
30歳以下における静注ドラッグ使用に関連したC型肝炎ウイルス感染症の増加 ― ケンタッキー州、テネシー州、バージニア州、ウェストバージニア州、2006~2012年
2006~2012年、ケンタッキー州、テネシー州、バージニア州およびウェストバージニア州におけるC型肝炎ウイルス(HCV)感染症のサーベイランスデータより、静注ドラッグ使用(IDU)によるHCV感染リスクについて分析した。この期間、急性HCV感染症例の報告数が全国的に増加しており、ミシシッピ川東部、とくに中央アパラチアにて顕著であり、これら4州にて1,377例が報告された。年齢の明らかな1,374例のうち、616例(44.8%)が30歳以下であり、301例が都市部[男:142例、女:155例、不明:4、非ヒスパニック系白人:249例(82.7%)]、315例が非都市部[男:157例、女:156例、不明:2例、非ヒスパニック系白人:247例(78.4%)]の症例であった。感染リスクは265例(43.0%)にて明らかであり、IDUによるHCV感染例は都市部にて99/138例(71.7%)、非都市部では95/127例(74.8%)であった。2006~2012年の間にHCV感染の発症率は都市部、非都市部にていずれも有意に上昇し、発症率はいずれの年においても非都市部にて都市部の2倍以上高値であった。オピオイド依存による治療入院率はこの期間にて21.1%上昇し、オピオイド処方薬、ヘロインによる入院率はそれぞれ16.8%、7.4%の増加であり、オピオイド静注による入院率は12.6%、非オピオイド静注による入院率は2.1%の増加であった。これらの地域における静注ドラッグによるHIV感染症の増加はまだ認められていないが、HCV感染症の増加と同様にHIV感染症の増加が懸念され、薬物依存の治療の必要性が示唆される。
