Concerns Regarding a New Culture Method for Borrelia burgdorferi Not Approved for the Diagnosis of Lyme Disease

Borrelia burgdorferiの新しい培養方法がライム病の診断法として承認されなかった事に関する問題

2005年、CDCおよびFDAはライム病の検査に正確性および臨床的有用性が確立していない方法が使用されていることに警告を発表した。これらの検査は、研究室にて開発された検査であり、単一施設にて使用され、FDAの承認は受けていないものが多い。CDCは最近、ライム病の原因となるスピロヘータBorrelia burgdorferiの培養法を利用して新しく開発された検査法に関する照会を受けた。この方法は2段階のpre-enrichment過程を利用してインキュベートした後、PCR分析による(またはPCR分析を行わない)免疫染色法にて判定するものであるが、CDCによる再調査では、誤診の原因となる偽陽性が認められた。CDCはFDAにより承認された2段階検査法(EIA法にて陽性または擬陽性の場合、ウエスタンブロット法を行う)を利用するよう推奨しており、臨床検体の培養またはPCR法は特殊な場合にのみ推奨される。
