Vital Signs: Evaluation of Hepatitis C Virus Infection Testing and Reporting － Eight U.S. Sites, 2005-2011

バイタルサイン:C型肝炎ウイルス感染症の検査および報告－アメリカ8地域、2005～2011年

アメリカにおけるC型肝炎ウイルス(HCV)感染生存症例は約3,200万人であり、うち45～85%が感染に気づいていないと推定されている。CDCは国内の8地域(コロラド州、コネチカット州、ミネソタ州、ニューメキシコ州、ニューヨーク市、ニューヨーク州、オレゴン州、サンフランシスコ)のHCVサーベイランスをサポートしており、今回、2005～2011年のサーベイランスデータの分析が行われた。この期間、新規にHCV感染が確認された症例は217,755例であり、うち107,209例(49.2%)はHCV抗体陽性、110,546例(50.8%)はHCV RNA陽性であった。HCV RNA陽性例は現在の感染例と考えられるが、HCV抗体陽性例は過去に感染し治癒した症例、偽陽性例および現在の感染例が含まれる。新規確認例のうち、HCV抗体陽性例が占める割合はニューメキシコ州:76%～ミネソタ州:23%であり、HCV抗体陽性、HCV RNA陽性例ともに1945～1965年生まれが多く、それぞれ58.5%、67.2%を占めた。死亡例は6,734例(/197,844例、3.4%)であり、年齢は50～59歳が最も多く(44.8%)、1945～1965年生まれは71.5%であった。HCV抗体陽性に占める死亡例の割合(4.6%)は、HCV RNA陽性例(2.4%)よりも有意に高かった。2011年の新規確認例の割合は人口10万人あたり全体で84.7(ミネソタ州:36.0～サンフランシスコ:239.2)であった。以上、新規にHCV感染が確認された症例のうち約半数はHCV抗体検査での診断であり、過去の感染例との判別がつかないことから改善されるべきであり、また、1945～1965年生まれにおける陽性率が高く、死亡例が多いことから、とくにこの年代を対象としたHCV感染の検査および治療を推進すべきと考える。
