HIV感染症の検査:臨床医および研究者に対するガイドラインの最新情報

CDCは1998年以降、HCV感染リスクがある場合は検査を受けるよう推奨しており、2003年に検査および結果報告に関するガイドラインを発表している。その後、抗体検査薬の有効性の変化、過去の感染例もHCV抗体検査が陽性となること、また、HCVに対する有効性が高い抗ウイルス薬が開発されたことなどを受け、2012年にガイドラインの修正を行った。修正されたガイドラインでは、予備診断としてHCV抗体の迅速検査(OraQuick HCV Rapid Antibody Test)を導入し、RIBA HCVの使用を停止し、Chiron RIBA HCV 3.0 Strip Immunoblot Assayの使用を推奨している。また、現在の感染例を識別するため核酸増幅検査(NAT)を実施し、迅速検査にて陽性の場合、FDAが認可するNATアッセイにより血清または血漿中のHCV RNAを検出する方法が推奨されている。また、急性C型肝炎およびC型肝炎は報告義務のある疾患であるため、感染が確認された場合は州および地域の法律に従って、CDCに報告される。HCV検査に関するガイドラインは現在も続けられているより精度および特異性の高い方法の開発状況により改訂されるものである。
