FDA Licensure of Bivalent Human Papillomavirus Vaccine (HPV2, Cervarix) for Use in Females and Updated HPV Vaccination Recommendations from the Advisory Committee on Immunization Practices (ACIP)

2009 年10月16日、FDAは10～25歳の女性用として2価ヒトパピローマウイルスワクチン(HPV2;Cervarix)を認可した。Cervarixはアメリカで女性用に認可された2番目のHPVワクチンである。2006年には9～26歳の女性用として4価HPVワクチン(HPV4;Gardasil) が認可されており、ACIPは11、12歳の女性に対してHPV4の定期接種、13～26歳の女性にキャッチアップ接種を勧告している。この報告は HPV2の臨床試験データと、女性に対するHPV2あるいはHPV4の定期・キャッチアップ接種の最新勧告(用量・用法、接種スケジュール、特殊な場合での接種、使用上の注意と禁忌)について述べる。両ワクチンともHPVの遺伝子組み換えL1カプシド蛋白質から調製されたウイルス様粒子(VLP)から構成されており、生ワクチンではない。HPV2は2種類の発癌性HPV(HPV16、18)、HPV4はそれに加えて2種類の非発癌性HPV(HPV6、 11)を含む。HPV2、HPV4はHPV16、18による子宮頸部上皮内腫瘍(グレード2/3)・上皮内腺癌の予防に有効であり(ワクチン効果:それぞれ92.9%、98.2%)、HPV4はHPV6、11による尖圭コンジローマ(性器疣贅)やHPV16、18による膣・外陰部上皮内腫瘍(グレード 2/3)の予防効果もある(それぞれ99.0%、100.0%)。FDAのHPV2認可に備えてACIPはHPV2とHPV4の免疫原性、有効性、安全性のデータをレビューし、2009年10月21日に女性におけるHPVワクチンの使用勧告を更新した。その勧告では、子宮頸癌・前癌の予防のため11、12 歳の女性に対してHPV2あるいはHPV4の3回接種(初回接種から1～2ヶ月後に2回目、6ヶ月後に3回目)を、ワクチン未接種または3回接種していない13～26歳の女性にキャッチアップ接種を勧めている。授乳婦への接種は可能だが妊婦への接種は勧められないため、接種後に妊娠が判明した場合には残りの接種は妊娠終了まで遅らせるべきである。HPVワクチンは、含有ワクチン成分や酵母に対する即時型アレルギーの発現歴のある者には禁忌である。
