1990～1993年に、公衆衛生に驚異的な変化が生じた。CDCは、学会、公衆衛生当局、民間による重要な連携を構築することにより、その効果と能力を拡大した。通信の激増は、CDCが研究結果および勧告を公衆と直接交信する能力を非常に高め、CDCと公衆衛生共同体の透明性を増大させた。CDCの優先順位は明確であり、公衆衛生インフラを強化し、小児の健康を改善し、国の医療制度における予防を実際に実現することである。21世紀を目前に、CDCは、感染症専門の卓越した研究拠点の必要性を初めて唱えた公衆衛生の先見者が見通した以上に複雑な課題に直面した。これらの優先順位を包括的にするには、展望と使命を明確にし、目標達成のための戦略を設定するCDCの新戦略計画のプロセスにかかっていた。より強力な公衆衛生システムの必要性は、医学研究所（IOM）による様々な科学報告書の中で述べられた。1990年、米国保健社会福祉省はHealthy People 2000の中で、これらの懸念を提起する国の枠組みである The National Health Promotion and Disease Prevention Objectivesを発表した。CDCは、これらの目標を展開するために国のリーダーシップを発揮し、22の Healthy People優先地域のうち12地域で主導機関あるいは共同主導機関として役目を果たした。 Healthy Peopleの目標を背景に、第一級の経営専門家による戦略計画のプロセスを使用し、CDCの世界に向けた展望が“Healthy People in a Healthy World”として“Through Prevention”を加えて詳細に述べられ、国の予防機関としてCDCの役割をより強調した。CDCの任務に関する声明も、疾病、負傷、身体障害の予防と管理によって健康で質の高い生活を促進することが明確に示された。CDCの未来設計を作成するにあたり、以下の5つの横断的戦略が明らかになった。
<ul<li提携：地域、国、国際機関との新しい提携を構築する一方で、公衆衛生の協力者との連携を強める。<li政府改革：予防計画の統合と連携を働きかける。<li健康に関するコミュニケーションと社会的マーケティング：健康に関するメッセージを作成し、戦略的に発表するために消費者調査を使用する。<li効果：予防サービスと公衆衛生プログラムの有効性、効果、費用効果を評価する。<li職員の多様化：CDCが対象とする人々を反映した多様な人員を形成する。<
