エンテロウイルスサーベイランス－アメリカ、2002～2004年

エンテロウイルスは軽度の発熱性疾患から重度の致死的な無菌性髄膜炎等まで多様な臨床症状を引き起こす。エンテロウイルスの血清型は全部で68種類確認されており、個々の血清型は経時的に異なった循環パターンを示し、様々な臨床症状を発現させる。この報告では、National Enterovirus Surveillance System(NESS)のデータに基づきアメリカにおける2002－2004年のエンテロウイルス感染症の動向を紹介する。2002－2004年、24の研究所が46州およびプエルトリコにおいてエンテロウイルス4,123株を検出したと報告した。この期間中にアメリカで検出されたエンテロウイルスの半数以上をエコーウイルス9型とエコーウイルス30型が占めていた。エコーウイルス9型は41州とプエルトリコ、30型は38州とプエルトリコで確認された。検出源としては脳脊髄液が最も多かった。ウイルス検出症例の年齢は生後1ヶ月未満－95歳であった。エコーウイルス9型、30型が最も流行した血清型としてNESSにより同定されたことは、アメリカで2003年に多発したそれらの血清型による無菌性髄膜炎のアウトブレイクの調査に役立った。エンテロウイルスは個々の血清型が経時的に循環し、流行する血清型も変化するため、大規模なアウトブレイク引き起こす可能性があり、そのモニタリングは重要である。2000年にCDCおよび各州の検査機関がエンテロウイルス検体の結果をNESSに直接報告するようになり、それ以降エンテロウイルスの検出率は増えている。エンテロウイルスサーベイランスにおいて、特に各州の衛生研究機関の参加が増えることは有益であり、今後も報告は増加してゆくと考えられる。
