Assessing Capacity for Surveillance, Prevention, and Control of West Nile Virus Infection － United States, 1999 and 2004

西ナイルウイルス感染症のサーベイランス、予防および制御の能力評価－アメリカ、1999年と2004年

アメリカにおいてヒト西ナイルウイルス(WNV)による疾患は、1999年8月にニューヨーク市で最初に確認されてから2004年末までに、ワシントン州、ハワイ州、アラスカ州を除く全ての州でその発生が報告されている。ヒトへのWNV感染経路は5経路(主にCulex種の蚊に刺される、輸血、臓器移植、経胎盤移行、授乳)存在する。アメリカにおける1999－2005年のWNV疾患発症例は全体で19,525例であり、771例が死亡している。CDCは2000年にWNVのサーベイランス・予防・制御ガイドラインを初めて発表し、電子サーベイランス・報告システムであるArboNETを創設した。また1999年初めに一部の州と大都市で開始されたWNVサーベイランス・予防活動はその後50州、6大都市/郡、プエルトリコに拡大した。州および大都市/郡の保健機関におけるWNVサーベイランス・予防・制御活動の能力を評価するため、2005年にCouncil of State and Territorial Epidemiologists(CSTE)はAssociation of Public Health Laboratories(APHL)およびCDCと協力し、50州および6大都市/郡の保健機関におけるWNVプログラムを調査し、49州および6大都市/郡が回答した。そのうち40州(82%)は感染症専門家に疾患報告を行うよう伝えており、また31州(63%)および27州(55%)はそれぞれ、入院を要する病因不明の脳炎および髄膜炎の報告も義務付けていた。48州中32州(67%)は検査ベースのサーベイランスシステム［アルボウイルス感染症確認のための脳脊髄液(CSF)検査］を実施していた。さらに49州中46州(94%)は蚊のサーベイランス、41州(84%)は馬の神経学的疾患の報告、28州(57%)は死亡した鳥類の情報収集を行っていた。検査施設に関しては、47州中41州(87%)がCDCガイドラインで推奨されているバイオセーフティ・レベル3(BSL-3)の設備があることを報告した。また回答した47州全てがELISA法を用いてWNV特異的IgMの同定を行っていた。予防活動としては、ほぼ全ての州のプログラムでWNV予防メッセージ［DEET(N,N-ジエチル-m-トルアミド)の使用、住宅地での蚊のコントロールなど］が配布された。本調査により、WNVのサーベイランスシステムや検査機関の能力、予防プログラムは1999年以降かなり進歩していることが明らかになった。
