ポリオ根絶への進展－ナイジェリア,2004年1月～2005年7月

1988年以降の世界的なポリオ根絶運動によりポリオが風土病である国の数は1988年の125カ国から2003年には6カ国にまで減少した。しかし2003－2004年、ナイジェリアにおいてワクチンの安全性に関する誤解が原因で北部の州での追加予防接種活動(SIA)が一時的に停止した後、国内で野生株ポリオウイルス(WPV)伝播が増加した。このナイジェリアでのポリオ復活により、2003年から2005年にかけて1年以上ポリオフリーであった18カ国にWPVが拡大した。この報告は、2004年1月－2005年7月のナイジェリアにおけるポリオ根絶活動とWPV伝播状況、WPV伝播遮断活動についてまとめたものである。2004年、ナイジェリアは全37州をターゲットとして全国予防接種日(NID)を5回実施し、北西部の8州では7月と9月に準全国予防接種日(SNID)を実施した。2005年にはNIDが5回、SNIDが1回、計画された。2004年の急性弛緩性麻痺(AFP)サーベイランスでは、非ポリオAFP症例発生率が7.3(目標:15歳未満の小児10万人あたり2例以上)であり、全37州の100%、全774地方自治体の65%が目標に到達した。また適切な便検体採取率は91%(目標:80%以上)であり、全州の95%、全自治体の78%が目標に到達した。ナイジェリアで確認されたWPV症例数は2002年202例、2003年355例、2004年782例(WPV1型:600例、3型:182例)と増加した。このうち717例(92%)は3歳未満であり、全例がワクチン未接種か接種が不十分であった。ナイジェリア政府とその関連機関は、SIAの質の改善や定期的なワクチン接種サービスの強化など、WPV伝播遮断に向けた様々な努力を実施中である。
