Serious Psychological Distress Among Persons with Diabetes - New York City, 2003

糖尿病患者における重度の精神的苦痛－ニューヨーク市,2003年

糖尿病患者では、重度の精神的苦痛（SPD）の原因となる鬱病、不安などの疾患がしばしばみられる。糖尿病成人におけるSPDの有病率とその影響について評価するため、New York City Department of Health and Mental Hygiene（DOHMH）は2003年New York City Community Health Survey（CHS）に登録された18歳以上の成人9,590人のデータを分析した。対象とした9,590人のうち498人（年齢調整有病率:5.0%）がSPDを有しており、857例（9.0%）は糖尿病患者であった。これら糖尿病患者のSPD有病率は10.4%（80/857例）であった。年齢、人種/民族、婚姻状態、世帯所得を調整後の糖尿病患者のSPD有病率は非糖尿病成人のほぼ2倍であった。SPDを有する糖尿病患者は糖尿病のみの患者に比べ別居や離婚、死亡により配偶者やパートナーを失っている場合が多く（49%vs25%）、貧困であり（世帯所得25,000ドル以下:70%vs43%）、健康状態不良との回答が多く（78%vs40%）、健康管理の受診率が低かった。SPDを有する糖尿病患者の健康状態を改善するためには、社会経済的バリアを焦点とし受診率を改善させるための一貫した身体的、精神的健康管理プログラムが必要と考える。
