Hepatitis B Vaccination of Inmates in Correctional Facilities - Texas, 2000-2002

矯正施設における収容者のB型肝炎ワクチン接種－テキサス州,2000～2002年

2002年12月、アメリカでは約220万人が拘束され、同年に約800万人が釈放された。2001年、アメリカでは約22,000例が急性B型肝炎を、78,000例が新たにB型肝炎ウイルス(HBV)感染症を発症したが、これら患者のうち約29%は拘束経験者であった。矯正施設内および拘束者が社会復帰したときのHBV感染症伝播予防のために、拘束者へのB型肝炎ワクチン接種が勧められている。2000年5月、州の刑務所および拘置所収容者の管理監督を行っているTexas Department of Criminal Justice(TDCJ)はB型肝炎ワクチン接種プログラム(2ヶ月間隔での3回接種)を実施した。2000―2002年の拘束者におけるB型肝炎ワクチン接種率を調査するため、TDCJはワクチン接種歴やHBV感染歴のないカルテを再検討した。B型肝炎ワクチンは426名中319名(75%)に接種されており、刑務所入所者［185/220名(84%)］は拘置所入所者［134/206名(65%)］よりも接種率が高かったが、これは拘置所における収容者の回転率の高さとスタッフとの接触時間の欠如が関与していると考えた。しかしワクチン1回接種率は拘置所入所者［114/134名(85%)］が刑務所入所者［134/185名(72%)］より高率であった。ワクチン接種を開始し4ヶ月以上拘束された刑務所収容者125名、拘置所収容者99名における3回接種完了率は、それぞれ96%、54%であった。2002年12月、B型肝炎ワクチン接種プログラムは資金難のため一時中断された。刑務所および拘置所におけるB型肝炎ワクチン接種プログラムの確立により、地域社会の成人におけるHBV感染をかなり予防できる。ワクチン接種プログラム実施の障壁を克服するためには、公衆衛生機関と矯正機関との協力が不可欠である。
