Tuberculosis Associated with Blocking Agents Against Tumor Necrosis Factor-Alpha - California, 2002-2003

TNF-α阻害薬による結核－カリフォルニア州,2002～2003年

FDAは、TNF-α拮抗薬のインフリキシマブ(Remicade)、エタネルセプト(Enbrel)、アダリムマブ(Humira)による治療の有害反応として結核(TB)疾患が発症する可能性があることを明らかにした。これら薬剤は炎症性サイトカインであるTNF-αの阻害作用を有し、慢性関節リウマチや他の自己免疫疾患の治療に承認されている。TNF-αは一部の感染性疾患、特にTBの病態生理などに関連しており、TNF-αの阻害は潜在TB感染症(LTBI)を顕在化させる可能性がある。2002年、カリフォルニアの郡健康局はインフリキシマブ治療に関連したTB疾患3例を報告した。この報告ではこれら3例とその後報告された9例の概要を紹介し、TNF-α阻害薬の投与を受けている患者のための暫定的な勧告を示す。カリフォルニアで報告された3例は慢性関節リウマチの55歳男性と64歳女性、クローン病の54歳女性であり、それぞれインフリキシマブによる治療を17、9、16ヶ月間受けた後発熱や体重減少などが発現し、肺結核、心膜TB疾患、肺結核と診断された。2003年、California Department of Health Servicesは2002年1月以降に発症したTNF-α拮抗薬関連TB患者を報告するよう要請した。2003年9月現在、さらに9例が報告され、2002年1月―2003年8月のTB疾患診断例は12例となった。これら患者の年齢中央値は54.5歳(23―73歳)、8例(67%)が女性であった。11例はインフリキシマブ治療後、1例は長期エタネルセプト治療後にTB疾患が発症した。11例はLTBIの危険因子(TB流行国での出生、TB疾患患者との接触など)を最低1つ有していた。また8例はTB診断時、他の免疫抑制療法を受けていた。健康管理者は免疫不全患者に対してTB予防策を実施する必要があり、説明できない熱性疾患の原因としてTBに注意するべきである。
