Cancer Distribution Among Asian, Native Hawaiian, and Pacific Islander Subgroups — United States, 2015–2019

非ヒスパニック系アジア人(アジア人)と非ヒスパニック系ネイティブハワイアン/太平洋諸島民(NHPI)は、アメリカ人口の中で増加を示している。がんの疫学研究では、しばしばアジア人とNHPIは一つにまとめて集計されるが、アジア人およびNHPIは文化的、地理的、言語的に多様であるため、サブグループ解析により健康転帰の分布に関する見識をもたらす可能性がある。CDCは、アジア人およびNHPIの25サブグループにおける新規がん症例の頻度およびと割合を調査するために、最新の2015～2019年のU.S. Cancer Statisticsを解析した。アメリカでは2015年～2019年に、アジア人で合計273,656例、NHPIで合計18,491例の新規浸潤がん症例が報告され、その中には東アジア人92,562例(アジア人およびNHPIの全症例の31.7％)、東南アジア人の71,721例(同24.5％)、南アジア人の44,890例(同15.4％)、その他の地域のアジア人64,483例(同22.1％)が含まれていた。アジア人およびNHPIサブグループにおける新規がん症例の分布は、性別、年齢、がんの種類、診断時のステージ(スクリーニング検出で検出されたがんの場合)により異なり、症例の約半数が女性(アジア人56.2％、NHPI 56.5％)、約10分の1が40歳未満であった(アジア人8.5％、NHPI 9.6％)。新規がん症例で女性の占める割合が高かったのはタヒチ人(68.2％)、タイ人(65.5％)、フィジー諸島人(65.1％)のサブグループで、最も低いのはラオス人であった(47.1％)。40歳未満の新規がん症例の割合は、モン族(20.2％)、不特定の(NOS)ミクロネシア人(18.1％)、NOSメラネシア人(15.7％)で高く、日本人(3.1％)で最も低かった。最も多いがんの種類は25のサブグループでさまざまであり、例えば、乳がんは18のサブグループで多かったが、肺がんはチャモロ人、NOSミクロネシア人、ベトナム人で多く、大腸がんはカンボジア人、モン族、ラオス人、パプアニューギニア人で多かった。すべてのサブグループにおける末期のステージ診断の割合は、乳がんが25.7％～40.3％、子宮頸がんが38.1％～61.1％、大腸がんが52.4％～64.7％、肺がんで70.0％～78.5％であった。サブグループのデータはアジア人/NHPIにおける医療格差を示しており、健康の社会的決定要因に対処するプログラムをはじめとする文化的および言語的に対応したがんの予防およびコントロールプログラムの設計と実施を通じて、医療格差を縮小できる可能性がある。
