COVID-19 Outbreak at an Overnight Summer School Retreat — Wisconsin, July–August 2020

宿泊型サマースクールリトリートにおけるCOVID-19アウトブレイク ― ウィスコンシン州、2020年7月～8月

2020年7月2日～8月11日、ウィスコンシン州にて男子の宿泊型サマースクールリトリートで新型コロナウイルス感染症(COVID-19)アウトブレイクが発生した。リトリートには、21の州およびアメリカ領土と海外2カ国から152名の男子高校生、カウンセラー、スタッフが参加していた。すべての参加者は、過去3カ月以内の血清学的検査結果か、旅行前7日以内のRT-PCR検査によるSARS-CoV-2(COVID-19の原因ウイルス)陰性結果のいずれかの証明書の提出、旅行前7日間は家庭内で自主隔離、旅行中のマスク着用が要求された。7月3日に、1週間以内のRT-PCR検査結果が陰性であった9年生(インデックス患者)が、咽頭痛、咳、悪寒を発症し、7月5日にRT-PCR検査結果陽性が判明した。この生徒が出発した約1週間後に、家族のRT-PCR検査結果が陽性となったことが後に判明した。この生徒はリトリートで個室に隔離され、濃厚接触者11名が別の大部屋で一緒に隔離された。7月15日に、ウィスコンシン州保健局（WDHS）は、リトリートの2人の生徒がSARS-CoV-2のRT-PCR検査結果が陽性であるとの通知を受け、調査を開始した。WDHSは7月28日に参加者にRT-PCR検査を、8月5日と6日に血清学的検査を提供した。78人(51％)の参加者がRT-PCR結果が陽性で(確認された症例）、38人(25％)がRT-PCR結果の陽性なしにCOVID-19の臨床基準を満たした(可能性のある症例)。リトリートの終了までに、118人(78％)が血清学的検査で陽性となった。リトリート前に血清学的検査結果が陽性であった24人の参加者では、全員がRT-PCRの結果が陰性であった。7月28日のRT-PCR検査の後、WDHSは、残りの感染しやすい集団(無症候性でRT-PCR検査結果が陰性）を他の生徒やスタッフから隔離することを推奨した。隔離または検疫の終了日は、確立されたガイダンスに基づいて、CDCと調整して決定された。すべての出席者は、8月11日に帰国するための州間および民間航空機での旅行が許可された。今回のアウトブレイク調査から、SARS-CoV-2は、1人の生徒から、集合的な環境の青年と若年成人の間に急速に蔓延した可能性がある。これらの環境でのCOVID-19のアウトブレイクを防ぐには、到着前の検疫と検査、コホーティング、症状の監視、症例の早期発見と隔離、マスクの使用、強化された衛生と消毒の実践、最大限の屋外プログラミングを含む緩軽減計画が必要である。
