Advisory Committee on Immunization Practices Recommended Immunization Schedule for Adults Aged 19 Years or Older — United States, 2020

Advisory Committee on Immunization Practicesにより推奨される19歳以上の成人の予防接種スケジュール ― アメリカ、2020年

Advisory Committee on Immunization Practices(ACIP)は2019年10月の会議で、アメリカにおける19歳以上の人を対象とした2020年の推奨される成人予防接種スケジュールの承認の採決を実施した。2020年の成人の予防接種スケジュールはウェブサイト(https://www.cdc.gov/vaccines/schedules/index.html)で見ることができ、ACIPの推奨事項を2つの表と付随する注記によりまとめている。この2020年の成人予防接種スケジュールは、CDC所長、アメリカ内科学会、アメリカ家庭医学会、アメリカ産科婦人科学会、アメリカ助産師協会にも承認されている。医療従事者には、表と注記を一緒に使用することを助言する。今回の改訂には、A型肝炎ワクチン(HepA)、ヒトパピローマウイルスワクチン(HPV)、インフルエンザワクチン、血清群B型髄膜炎菌ワクチン(MenB)、肺炎球菌ワクチン、破傷風トキソイド+弱毒化ジフテリアトキソイド+無細胞性百日咳ワクチン(Tdap) に関する新規、または更新されたACIP推奨事項が含まれる。たとえばA型肝炎では、慢性肝疾患の定義の軽微な変更、妊婦への適応症の軽微な変更、曝露の状況における予防接種の推奨の追加、予防接種の適応から凝固因子障害の除外などの注記が改訂された。HPVでは、26歳以下のすべての人にHPVワクチン接種推奨に改訂され、27～45歳の人には、臨床判断を細分化することが加わった。インフルエンザワクチンの注記では、弱毒生インフルエンザワクチン(LAIV)を使用すべきでない場合の箇条書きリストが更新され、ギランバレー症候群の既往歴のある人向けのガイダンスが軽微な改編された。MenBでは、MenB接種の適応として補体阻害剤ラブリズマブ使用を注記に含むように改訂された。髄膜炎菌性疾患のリスクが高くない16～23歳の青少年および若年成人では、臨床判断を細分化することが追記され、「特別な状況」では、初回シリーズの1年後にMenBをブースター接種し、リスクが残っている場合は2～3年ごとに再接種することを推奨している。
