心臓病の小児と特別な医療ケアの必要性 ― アメリカ、2016年

2016年のNational Survey of Children’s Healthのデータを用いて、0～17歳の小児における心臓病の有病率の推定と特別な医療の必要性について分析した。364,150世帯をサンプルとして抽出し、138,009名(37.9％)の親にスクリーニング調査を実施、50,212名(36.4％)で本調査が完了した。そのうち、情報に不備がある1,733名を除いた48,479名において、心臓病が「現在ある」、「過去にあった」、「ない」小児はそれぞれ634名、498名、47,347名であり、アメリカの0～17歳の小児全体ではそれぞれ約90万人(1.3％)、75万5千人(1.1％)、6,810万人(97.6％)にあたると推定された。心臓病が現在ある小児の58.3％は男児、55.7％は非ヒスパニック系白人であり、21.5％は世帯収入が連邦貧困レベル(FPL) の100％未満、64.8％は両親が高卒以上の学歴、72.3％は両親のそろった家庭であり、55.4％は民間の医療保険に加入していた。心臓病の状態による人口統計学的な差は認められなかったが、心臓病が現在、または過去にあった小児のそれぞれ67.2％、60.5％が他の病気もあるのに対し、心臓病のない小児での割合は46.7％であった。また、心臓病が現在、または過去にあった小児のそれぞれ60.0％、40.0％が特別な医療ケアを必要としており、薬物の処方(それぞれ42.8％、26.6％)、平均以上の医療、メンタルヘルス、教育サービスの利用(41.8％、23.9％)などが多かった。心臓病が現在ある、または過去にあった小児では心臓病のない小児(18.7％)に比べ、それぞれ3.1倍、2.1倍多く特別な医療を必要とし、機能的制限に対するケアの必要性は心臓病のない小児との差が最も大きかった(それぞれ6.3倍、3.7倍)。心臓病が現在ある小児のうち、特別な医療を必要とする小児は男児、世帯収入がFPL100％未満の小児、両親が揃っていない家庭の小児にて多かった。以上、心臓病のある小児は特別な医療ケアを必要とするため、公的医療の資金計画にこれらのサーベイランスおよびスクリーニングの確立が重要であることが示唆される。
