新規のワクチン導入の状態 ― 全世界、2016年9月

1974年に世界的な予防接種拡大計画(EPI)が開始して以来、6つの疾患(結核、ポリオ、ジフテリア、破傷風、百日咳、麻疹)に対する予防接種は何百万の死亡および身体障害を予防している。ワクチンの開発および導入では大幅な飛躍がなされており、今や認可されたワクチンは25の疾患の予防に利用可能である。歴史的に、低所得および中所得国では、新規のワクチンは高所得国で導入され以降の10年間に利用可能になってきた。しかし、ワクチンの事前資格審査および調達を支援するWHOおよびユニセフをはじめとするグローバルパートナーの援助、同様に、低価格のワクチンと引き換えに、低所得国での需要の予測および供給を介して、財政的支援の提供およびワクチン市場の形成をするGAVIアライアンス(GAVI、ワクチンと予防接種のための世界同盟)の支援により、現在のワクチンの導入は、より迅速になっている。今回、2016年9月現在のWHO加盟国において、インフルエンザ菌b型(Hib)、B型肝炎、肺炎球菌結合型、ロタウイルス、ヒト・パピローマウイルス、風疹の各ワクチンおよび麻疹ワクチンの2回目の接種の定期予防接種への導入状況を、WHO Immunization Vaccines and Biologicals Databaseを使用して調査した。2016年9月現在、194のWHO加盟国のうち、乳幼児のワクチン接種スケジュールに導入されたのは、Hibワクチン：191カ国(99％)、B型肝炎ワクチン：190カ国(98％)、肺炎球菌結合型ワクチン：132カ国(68％)、ロタウイルスワクチン：86カ国(44％)であった。ヒト・パピローマウイルスワクチンは、主に思春期の少女での定期使用を目標に67カ国(35％)で導入されている。また、麻疹含有ワクチンの2回目の接種は161カ国(83％)で、風疹ワクチンは149カ国(77％)で導入されている。これらの取り組みは、全ての人へ予防接種の完全実施を拡大するために2012年に世界保健総会により承認された、Global Vaccine Action Plan 、2011～2020年に概説されているコミットメントを支援している。
