HIV Infection and HIV-Associated Behaviors Among Persons Who Inject Drugs — 20 Cities, United States, 2012
注射薬物使用者におけるHIV陽性率およびHIV関連リスク行動 ― アメリカ20都市、2012年
2012年、注射薬物使用者(PWID)におけるHIV陽性率およびHIV関連リスク行動について調査するため、CDCはAIDS有病率の高い20都市においてNational HIV Behavioral Surveillanceのスタッフによるインタビュー調査を実施した。PWID計10,002名が調査に協力し、そのうち11%がHIV陽性であった。陽性率は非ヒスパニック系黒人(16%)が非ヒスパニック系白人(5%)に比して高く、南部地域(13%)が中西部(8%)および西部(7%)地域より高かった。HIV陽性率はヘロインのみの常用者で7%と、ヘロイン以外または複数薬物種使用者の17%に比べ低かった。男性のPWIDのうち、過去12カ月以内に男性間性交渉を行った者の陽性率は27%であった。また、HIV陽性PWIDのうち、63%は陽性を自覚していた。PWID 9,425名の行動分析の結果、過去12カ月以内に30%が注射器を共有、70%が無防備な膣性交、25%が異性間の無防備な肛門性交をし、49%が複数の異性の性交パートナーを有していた。男性PWIDの10%が男性間性交渉を報告、5%が12カ月以内に無防備な男性間性交渉を行っていた。過去12カ月以内に、PWIDの25%がHIVリスク行動教育を、39%が薬物中毒の治療を、51%がHIV検査を受けていた。78%が過去にC型肝炎ウイルス検査を受けていた。2012年のPWIDのHIV陽性率(11%)および陽性自覚率(63%)は2009年(9%および55%)と有意差はなく、リスク行動率も同程度であった。今後、PWIDにおけるHIV感染を低減するために、リスク行動の教育、HIV検査の受診率向上、薬物治療など予防プログラムの努力が必要である。
