Prevalence and Indicators of Viral Suppression Among Persons with Diagnosed HIV Infection Retained in Care — Georgia, 2010

HIV感染症と診断され、治療中の患者におけるウイルス抑制率とその指標 ― ジョージア州、2010年

2012年3月、アメリカにおけるHIV治療ガイドラインは、全ステージのHIV患者に対して抗レトロウイルス療法(ART)が推奨されると変更された。Georgia 州のDepartment of Public Healthはこのガイドラインの実施に先立ち、HIV患者におけるウイルス抑制に対する診断時の疾患ステージの影響について検討した。2010年、ジョージア州における新規HIV感染患者(13歳以上)は2,921例であり、うち1,340例(45.9％)は治療を継続、治療継続例は黒人が多く(53.7％)、年齢は13～24歳:19.0％、25～34歳:27.3％、35～44歳:22.9％、45～54歳:21.6％、55歳以上:9.0％、診断時の疾患ステージは1:15.8％、2:25.1％、3:28.0％、不明:31.2％であった。男性(1,033例)における感染経路は同性間性交渉(61.7％)、女性(307例)では異性間性交渉(67.9％)が最も多かった。診断から15ヵ月以内にウイルス抑制の基準(ウイルス量200 copies/mL未満)を満たした症例は958例(71.5％)であり、抑制率は黒人(63.3％)、13～24歳(56.5％)にて低く、診断時のステージ別ではステージ1:64.5％、2:72.3％、3:80.5％、不明:66.3％であった。人種/民族、年齢および診断時の疾患ステージは、診断から15ヵ月以内のウイルス抑制の予測因子として有意性を示し、黒人、13～24歳、診断時ステージ1およびステージ2の症例にてそれぞれ白人、55歳以上、ステージ3の症例に比べ低かった。これらの結果からHIV患者に対するARTによるウイルス抑制は全てのステージの患者に対しても有効であることが示唆された。
