Trends in Uninsured Clients Visiting Health Centers Funded by the Title X Family Planning Program — Massachusetts, 2005–2012

Title X family planning programが資金援助する健康センターを訪れる医療保険非加入者の動向 ― マサチューセッツ州、2005～2012年

2006年、マサチューセッツ州は法律により住民の医療保険の加入を拡大し、医療保険非加入率は2006年、非高齢者にて約9～20％であったが、2011年には3％にまで低下していることから、医療保険非加入者に対する公的プログラムの必要性が疑問視され始めている。ここでは州内の6つの医療機関において医療保険非加入者が地域の医療機関で連邦政府の補助金によるプログラム(Title X family planning program)を受けるクライアントの動向について報告する。2005～2012年、マサチューセッツ州にてTitle-Xが費用を負担したクライアントのうち、医療保険非加入者の割合は59％(46～77％)から36％(24～52％)に減少しており、年齢別、世帯収入別でも同様に低下していた。20～29歳はこれらの機関を受診した医療保険非加入者の43～46％を占めており、また、非加入率の低下が最も大きかった(2005年:65％、2012年:39％)。また、世帯収入別では連邦貧困基準の101～138％および139～250％の非加入者がそれぞれ46％、43％を占めていた(2012年)。以上、2005～2012年におけるTitle-Xプログラムのクライアント数は変化なく、医療保険非加入者の占める割合は低下していたが、Title-Xプログラムは医療保険の加入にかかわらず必要とされるものであると思われた。
