Spinal and Paraspinal Infections Associated with Contaminated Methylprednisolone Acetate Injections － Michigan, 2012-2013

汚染された酢酸メチルプレドニゾロン注射液による脊髄および傍脊髄感染症－ミシガン州、2012～2013年

2012年9月18日、テネシー州にて汚染された酢酸メチルプレドニゾロン(MPA)注射液(New England Compounding Center製造)による真菌性髄膜炎の発症が報告され、その後、アウトブレイクは拡大し、2013年5月6日には20州にて741例、死亡例は55例となり、ミシガン州では計261例、死亡例は16例となった。ミシガン州では4ヵ所の疼痛治療施設が汚染されたMPA注射液3ロット、約17,000バイアルのうち2,225バイアルを受け取った(これらのロットは現在、全てNew England Compounding Centerにより自主回収されている)。汚染されたMPAは少なくとも1,791例に対し2,537回投与された。2013年1月29日の時点でデータの得られた180例(男75女105、年齢中央値:65歳)の分析では、141例にて髄膜炎を伴わない脊髄または傍脊髄感染症(I群:男61女80、中央値:65歳)、39例にて髄膜炎を伴う脊髄または傍脊髄感染症(II群:男14女25、中央値:67歳)が発症、4例(2%)が死亡した。主な症状はI群では腰痛(71%)、頭痛(35%)、頸部痛または頸部硬直(21%)が多く、II群では頭痛(72%)、悪心/嘔吐(46%)、腰痛(46%)が多く認められた。MPAの投与は1回が多く(I群:66%、II群:79%)、最終投与からMRIによる感染の確認までの期間は中央値50(12～121)日[I群:51(12～121)日、II群:46(23～116)日]であった。
