低所得者層の就学前小児における肥満率－アメリカ、1998～2008年

小児肥満は低所得者層の少数民族小児における主要な公衆衛生上の問題である。就学前に肥満であった小児は青年期や成人期に肥満となる可能性が高く、糖尿病や高血圧症、高脂血症、喘息、睡眠時無呼吸の発症リスクも高い。Healthy People 2010では、小児と青年の肥満率を5%に低下させることがひとつの目標となっている。CDCはPediatric Nutrition Surveillance System(PedNSS)の1998～2008年のデータを用いて、低所得者層の就学前小児(2～4歳)を対象とした小児肥満率の傾向と現状を分析した。肥満は、BMIが2000年のCDC性別成長チャートに基づいた年齢別BMIの95パーセンタイル以上と定義した。低所得者層の就学前小児における肥満率は1998年の12.4%から2003年には14.5%に増加したが、その後安定し、2008年の肥満率は14.6%であった。人種/民族別肥満率は 1998～2003年にはアジア/太平洋諸島系(A/PI)小児以外全ての人種/民族群で増加したが、2003～2008年にはアメリカンインディアン /アラスカ原住民(AI/AN)小児以外全ての群で安定していた。2008年の肥満率はAI/AN(21.2%)とラテンアメリカ系(18.5%)が高く、非ラテンアメリカ系白人(12.6%)と非ラテンアメリカ系黒人(11.8%)、A/PI(12.3%)が低かった。小児肥満の減少には、身体活動と健康的なダイエットを促進するための環境と対策を焦点とした家庭・小児保健施設・コミュニティにおける効果的な予防戦略が必要と考える。
