高校のフットボールチーム選手におけるメチシリン耐性黄色ブドウ球菌－ニューヨーク市、2007年

2007 年9月12日、New York City Department of Health and Mental Hygiene(DOHMH)にブルックリン高校のフットボールチームに所属する3名が培養検査によりメチシリン耐性黄色ブドウ球菌(MRSA)皮膚および軟組織感染症(SSTI)と診断されたと報告された。このチームは8月19～24日、学校の体育館にて59名の選手が参加して合宿を行っおり、うち51 名を対象としたDOHMHの調査により確診例4例と疑診例2例が確認された(罹患率:11.8%)。発症は8月24日～9月6日、確診例4例は足首、ふくらはぎ、臀部および足の甲に疱疹が出現、疑診例のうち1例は上腕部の膿痂疹、もう1例は腕および肘に膿痂疹が出現、全員がそれぞれの主治医に治療を受け、うち3例は切開と排膿処置を行った。投与された抗生物質は確診例3例にてバクトリム+ドキシサイクリン、クリンダマイシン+ムピロシン、バクトリム+ムピロシン、疑診例1例にてシプロフロキサシンであった。また、この6例のBMIは平均29.1と非感染例の平均値23.8に比べて有意に高かった。選手への聞き取り調査では、33名(65%)が合宿、レギュラーシーズン中に少なくとも1回以上の切り傷、擦過傷、芝熱傷を負ったが、軽症であったため自分で洗い、包帯を巻いていた。レギュラーシーズン中は練習後のシャワーは帰宅後なのでタオルの共用はなかったが、合宿中は10名(20%)がタオルを、6名 (12%)が石鹸を共用しており、感染リスクに関する多変量解析ではこのタオルの共用とBMI高値が感染リスクと高い相関を示した。
