輸血サービス強化に向けた進展-14ヶ国、2003~2007年
サハラ砂漠以南のアフリカでは、輸血が特に女性や子供における重大なHIVの感染源となっている。2004年以降、米国President’s Emergency Plan for AIDS Relief(PEPFAR)はHIV感染率の高いアフリカとカリブ海域の14ヶ国(ボツワナ、コートジボワール、エチオピア、ガイアナ、ハイチ、ケニア、モザンビーク、ナミビア、ナイジェリア、ルワンダ、南アフリカ、タンザニア、ウガンダ、ザンビア)において国内輸血サービスを強化するための技術的、財政的支援を行っている。PEPFARの支援内容としては、輸血の適切性と安全性を改善するための施策の指導や基盤となる研究施設の強化、リスクを有する献血者を除外するための関係職員の訓練や質問票の標準化などがある。PEPFAR支援を受けたこれらの国々における進展状況を評価するため、CDCは 2003~2007年に14ヶ国の国内輸血サービスで収集されたデータを分析した。2007年までに14ヶ国中12ヶ国が輸血の適切性と安全性に関する国家政策を制定した。また採取された全血ユニット数は、この期間中すべての国で増加した。人口1,000人あたりの採血率は11ヶ国で増加し、南アフリカとボツワナはWHOの推奨レベル(人口1,000人あたり10~20全血ユニット)に到達した。さらに報酬を受けない自発的ドナーからの採血率が100%である国は2003年の5ヶ国から6ヶ国に増え、他の6ヶ国はその割合が増加した。採取された血液ユニットのHIV陽性率は14ヶ国中13ヶ国で低下した。 HIV感染率の高い14ヶ国では、PEPFARイニシアチブの開始以降、安全で適切な輸血に向けた改善が進んでいる。
