Pseudomonas Bloodstream Infections Associated with a Heparin/Saline Flush - Missouri, New York, Texas, and Michigan, 2004-2005

ヘパリン/食塩水洗浄に関連したPseudomonas血症－ミズーリ州,ニューヨーク州,テキサス州,ミシガン州,2004～2005年

2005年1月26日、CDCはミズーリ州の癌治療クリニックの患者におけるPseudomonas fluorescens血症4例の報告を受けた。全例で、埋め込み式中心静脈カテーテル内の血液凝固を防ぐためIV Flush社がシリンジ内に注入したヘパリン/食塩水による洗浄が行われていた。2月15日現在、州および地域の健康局とCDCは4州(ミズーリ州、ニューヨーク州、テキサス州、ミシガン州)でヘパリン/食塩水洗浄後に発症したPseudomonas属感染症36例を確認している。この報告では、現在進行中の調査の概要と推奨される可能性例の調査および管理法を紹介する。ミズーリ州では2004年12月29日―2005年2月1日にP.fluorescens感染症9例(32―72歳)、ニューヨーク州では2004年12月13日―2005年2月8日にP.fluorescens感染症12例(生後8ヶ月齢―18歳)、テキサス州では2004年12月12日―2005年2月5日にPseudomonas属血症14例(生後16日齢―86歳)、ミシガン州では2005年1月にP.fluorescens感染症1例が確認された。全例が中心静脈カテーテルを長期(一部は短期)留置しており、IV Flush社が調製したヘパリン/食塩水によりカテーテル洗浄を行っていた。培養検査の結果、カテーテルを通した血液やIV Flush社製の未開封のヘパリン/食塩水シリンジにおいてP.fluorescensの増殖を認めた。感染とは無関係に入院中の1例を除き、全例がカテーテルの除去や抗生物質による治療などにより治癒した。2005年1月31日、FDAはIV Flush社がシリンジ内に注入した全てのヘパリンあるいは食塩水の使用に対する警告を公布し、IV Flush社はこれらの製品のリコールを開始した。医療提供者は中心静脈カテーテルを使用する患者でPseudomonas属感染症を確認した場合には、IV Flush社製のリコールされたヘパリン/食塩水洗浄液を使用したかどうかを確認する必要がある。
