Varicella-Related Deaths - United States, January 2003-June 2004

2003年1月―2004年6月、CDCは水痘関連死8例(アリゾナ:2、メリーランド:2、アーカンソー:1、ニューハンプシャー:1、オハイオ:1、ニューヨーク市:1)の報告を受けた。これら8例の年齢は1―40歳で、6例は20歳未満の小児および青少年であった。6例はワクチン未接種例であり、2例は接種状態不明であった。この報告では、これら死亡例のうちアリゾナ州、アーカンソー州、ニューヨーク市から報告された小児3例の臨床データを紹介する。これら3例は2003年10月に発症した12歳男児、2004年1月に発症した10歳女児、2004年3月に発症した生後14ヶ月齢女児であった。全例がワクチン未接種であり、このうち2例は水痘を発症したクラスメートと接触していた。また、この他の小児および青少年死亡例3例は免疫無防備状態であった。2003年の生後19―35ヶ月齢の小児における州別水痘ワクチン接種率は67―93%と様々であり、28州は接種率が85%未満である。水痘関連死の予防には、現在勧告されているように生後12―18ヶ月齢の小児に対するタイムリーなルーチンでのワクチン接種とより年長の易感染性小児・青少年に対するキャッチアップ接種を実施することが重要である。またタイムリーで完全な全国的水痘死サーベイランスも必要である。
