テロに対する炭疽病ワクチンの使用に関する予防接種諮問委員会の追加勧告

予防接種諮問委員会(ACIP)は2000年12月にテロ対策として炭疽病ワクチンの接種勧告を行い、暴露前接種および暴露後発症予防目的の接種(PEP)と抗生物質の服用を推奨した。2001年12月には、暴露危険性が高いと判断された場合(研究者など職務上反復暴露の可能性がある場合)の暴露前予防接種および研究室の安全レベルを”2″とするよう勧告した。その後PEPと抗生物質の併用による予防効果への疑問とワクチン供給の限界性から、ワクチン3用量レジメン(0,2,4週)がFDAへIND申請されたが、炭疽病予防の適応に対する許可は下りなかった。テロ発生後、約10,000名がB.anthracis暴露により60日間の予防的抗生物質投与を受け(完了率 : 42%)、それらの症例に炭疽病発症例は認められていない。現在ACIPはPEP非接種の場合は60日間の抗生物質投与、併用の場合は30日間の抗生物質投与を推奨している。小児や妊婦での安全性の検討、動物実験にてPEP併用/非併用による予防効果の検討が行われており、抗毒素など新しい治療法の研究も進められている。
