Recommendations of the Advisory Committee on Immunization Practices for Use of Hepatitis A Vaccine for Persons Experiencing Homelessness

A型肝炎(HepA)ワクチン接種は、12～23カ月齢の小児、A型肝炎ウイルス(HAV)感染のリスクが高い人、および免疫を得たい人に日常的に推奨されている。 HAV感染のリスクが高い人には、A型肝炎の流行が中等度以上の地域への国際旅行者、男性と性行為をする男性、注射薬および非注射薬の使用者、慢性肝疾患のある人、凝固因子障害のある人、研究室環境でHAVに感染した霊長類またはHAVを扱う人、および中等度以上の地域的流行の国からの国際的な養子との密接な接触が予想される人が含まれる。ホームレス経験者もまた、HAV感染や重症の感染に関連した転帰のリスクが高い。2018年10月24日に、Advisory Committee on Immunization Practices(ACIP)は、ホームレスを経験している1歳以上のすべての人に、HAVに対する定期的な予防接種を勧告した。ACIP肝炎ワクチン作業部会は、ホームレス経験者にワクチンを投与するためのエビデンスのシステマティックレビューを実施し、これには、予防接種に関連する利点と有害事象を評価する一連の基準が含まれた。 HepAワクチンは免疫原性が高く、免疫正常な成人では、95％を超える人が1回のワクチン接種を受けてから4週間以内に防御抗体を産生した。HAV感染症は、主に人から人への感染によるか、あるいは汚染された食物や水の摂取による糞口経路によって感染する。ホームレス経験者では、厳格な手指衛生のような、HAVへの曝露を防ぐための代替戦略の効果的な実施は、この集団の生活状況のために困難である。ホームレス経験者のためのヘルスケアサービスに定期的なHepAワクチン接種を統合することは、時間とともに潜在的にリスクのある人口規模を減少させ、その結果、大規模なアウトブレイクのリスクを低減することができる。
