ジカウイルス疾患に対する準備 ― ニューヨーク市、2016年

WHOアメリカ地域にて急速に拡散したジカウイルスはアメリカの医療供給システムに大きな影響を与えており、ニューヨーク市内の病院はCDCのガイダンスに従って予防および対応策を講じている。2016年9月21日の時点でニューヨーク州における研究室レベルでのジカウイルス疾患の確診例は流行地域への旅行者、その性的接触者および子宮内で感染した乳児など715例となっている。ここではNYC Health + HospitalsによるZika Preparedness and Response Action Planについて報告する。Zika Action Planは感染リスクのある症例を迅速に識別し、検査および必要な治療を提供し、確診例および疑い例に対しカウンセリングを行うことを目的とする。症例のスクリーニングはNYC Health + Hospitalsに救急搬送されたジカウイルス疾患流行国からの帰国者を対象とし、流行国はCDCガイダンスに基づく最新の情報により更新される。流行国への渡航歴、流行国への渡航者との性的交渉、発症前30日以内の輸血や血液製剤の投与および臓器移植、その他ジカウイルス疾患例との疫学的な関連性を考慮し、また、妊婦の場合のスクリーニングは別に定められている。検査はDepartment of Health and Mental Hygieneまたは特定の民間機関にて行われ、2016年4月～7月にて計729例(4月：29例、5月：69例、6月：314例、7月：317例)が検査を受けている。ジカウイルス疾患の研究室レベルでの確診例は増加を続けており、NYC Health + Hospitalsのスタッフ教育や州と地方の保健局の協力体制の整備などが進められている。
