CDC Grand Rounds：包括的な公衆衛生アプローチによる自殺の防止

アメリカでは2014年、10歳以上の約42,000名が自殺しており、自殺率は人口10万人あたり2000年：12.1、2014年：15.4と27％増加している。男女別では男性にて10万人あたり24.3、女性では6.8と男性にて約4倍高く、人種/民族別ではアメリカインディアン/アラスカ原住民(20.2)にて、非ヒスパニック系白人(10.5)の約1.9倍、非ヒスパニック系黒人(5.8)の約3.5倍、ヒスパニック系(5.5)の約3.7倍高値であった。また、年齢別では35～64歳にてもっとも高く、2000年から33％増加していた。自殺の防止は精神(うつ、不安、精神病およびアルコールや物質依存症)の治療アプローチに基づくが、自殺念慮や自殺企図を助長する環境に寄与する因子を焦点とした包括的で幅の広い公衆衛生的アプローチが必要である。CDCのNational Violent Death Reporting Systemデータでは、自殺には複数の因子が関与し(うつ、親しいパートナーの問題、健康上の問題、金銭的な問題や法律問題)、また、個人や家族の暴力(幼児虐待、ネグレクト、自殺の家族歴など)も関与する。2014年、18歳以上の自殺者は41,425名、救急外来で治療を受けた自傷者は375,530名、自殺企図者：1,120,000名、真剣に自殺を考えた人は9,436,000名に及ぶ。自殺防止の公衆衛生的アプローチはCDC-funded Injury Control Research Center for Suicide Preventionとしてロチェスター大学と共同し、州、地域、国レベルにて活動に関する情報を提供するほか、WHOと共同し自殺に関するメディアやオンライン報告に関するガイドライン(センセーショナルな見出しや言葉を禁止、自殺の場所や方法について詳細な報道および写真や映像を避ける、自殺念慮の兆候などについて報道する、自殺対策に関する相談機関の情報を提供するなど)を提示している。
