2007年12月～2009年7月のアメリカにおける深刻な経済不況の影響は、公衆衛生および地域の臨床医療環境にも及んでいる。2015年1～2月、National Association of County and City Health Officials Forces of Changeは、ハワイ州およびロードアイランド州を除く全地域の保健衛生局(LHD)のうち約3分の1にあたる948のLHDに対し、運営予算、職員の雇用および公衆衛生サービスの変化についてインターネットによる調査を実施し、690件(73％)の回答を得た。その結果、LHDのうち23％は前会計年度よりも予算が削減されたと報告し、27％は翌年度予算の減少を見込んでいた。2014年の1年間で職員は3,400名減少し[自然減2,100名(62％)および一時解雇1,300名(38％)]、2008年以降、総計で51,700名の雇用が失われた。職員数の減少は大規模(地域内人口が50万人以上)のLHDの61％、中規模(同5万～50万人)のLHDの41％、小規模(同5万人以下)のLHDの26％から報告があった。36％がサービスの縮小を報告し、予防接種(14％)、糖尿病スクリーニング(14％)、高血圧スクリーニング(11％)などのサービスが縮小された。35％のLHDにおいて地域医療機関の受け入れ患者数を縮小した一方、人口ベースの予防サービスである肥満予防サービス(24％)や、タバコ、アルコールおよび薬物防止サービスは23％のLHDで拡充された。LHDの58％が非営利病院と、61％が一次医療機関と連携し、統計や臨床データの相互利用などエビデンスベースの公衆衛生サービスを実施していた。以上、予算の削減や職員数の減少によりLHDの業務は縮小しており、地域の公衆衛生システムにおいて多様な役割を担うために他機関との連携が必要であることが示された。
