Travel-Associated Zika Virus Disease Cases Among U.S. Residents — United States, January 2015–February 2016

アメリカ居住者における旅行に関連したジカウイルス疾患症例 ― アメリカ、2015年1月～2016年2月

ジカウイルスは蚊が媒介するフラビウイルスで、太平洋諸島およびアメリカ地域での最近のジカウイルス疾患アウトブレイクは、妊娠の有害な転帰をはじめとし、新たな感染および臨床所見の様相が見出されている。一方、検査室で確認されたジカウイルス疾患の疫学的および臨床的所見に関するデータは限られている。2015年1月1日～2016年2月26日の間に、CDCでの試験に基づいて33州およびワシントンDCに居住する合計116例の感染が同定された。1例は満期出産乳児で、その母親は妊娠第1期中ブラジルにおいてジカウイルス疾患に罹患していた。残りの115例のうち、24例(21％)は2015年に、91例(79％)は2016年に発生し、75例(65％)は女性、年齢の中央値は38歳(3～81歳)、小児および18未満の青年は11例(10％)であった。115例のうち110例(96％)はジカウイルス流行中の地域(主にハイチ、エルサルバドル、ホンジュラス、グアテマラ)への最近の旅行を報告し、5例では渡航歴がなかったが、疾患の症状がある旅行者との性的接触があった。115例全例で臨床症状を報告し、主に発疹(98％)、発熱(82％)、関節痛(66％)、頭痛(57％)、筋肉痛(55％)、結膜炎(37％)であった。兆候または症状が2つ以上あったのは110例(96％)、3つ以上あったのは75例(65％)であった。入院は4例(3％)で、死亡例はなかった。旅行日がわかっている109例では、帰宅後1日(中央値、範囲：帰宅前37日～帰宅後11日)で発症した。ジカウイルス感染の臨床検査エビデンスは、RT-PCR試験陽性(28例、24％)および血清学的検査陽性(87例、76％)により確認された。2例は最近の詳細不明のフラビウイルス感染の血清学的エビデンスであったが、確認された症例との疫学的関連(垂直感染および性的接触)に基づいてジカウイルス疾患に分類した。医療関係者は症例、特に妊婦に対し、ジカウイルスの感染のリスクおよびその他の蚊媒介感染症を含めた予防策について教育する必要がある。また、ジカウイルス流行中の地域へ旅行した症例、および流行地域からの帰国が2週間以内で症状がある人とコンドームを使用しない性的接触があった症例において、急性の発熱、関節痛、結膜炎が発症した場合はジカウイルス疾患を考慮すべきである。
