CDC Grand Round:児童虐待防止による健康的な未来の構築

児童虐待は回避可能な悲劇であり、防止可能な公衆衛生上の問題である。2012年、アメリカでは身体的、性的、精神的およびネグレクトを含む児童虐待により1,593名の児童が死亡した。児童保護局への報告件数は推計340万件に達し、調査により70万人が虐待を受けたと確定されたが、潜在的な虐待件数はさらに多いと考えられる。2011年の児童および保護者を対象とした調査データから、毎年13.8％の児童が虐待され、25.6％が幼少期に何らかの虐待を経験することが示唆された。虐待は、児童に直接的な身体的および精神的脅威を与えるだけでなく、リスク行動につながる社会的、情緒的、認知的障害を与え、その結果、病気、外傷、身体障害、短命と生涯にわたって影響を及ぼす。成人17,000人以上を対象にした調査から、小児期の逆境的経験はその後の人生における負の転帰(うつ、自殺企図、アルコールまたは違法薬物乱用、喫煙、望まない妊娠、胎児死亡、性感染症、肥満、癌、糖尿病、虚血性心疾患、早死)の傾向増大に関連することが明らかになった。CDCの推計では、2008年の新規児童虐待による生涯経済損失は12,400万ドルに上り、その69.2％が生涯にわたる生産性低下、20.2％が医療保健、3.7％が特殊教育、3.6％が児童福祉、3.2％が刑事裁判に係る費用であった。児童虐待は発生前の防止が重要であり、児童の健全な発達と公衆衛生の促進に向けた対策の実施が必要である。CDCは、児童虐待防止に関する調査研究、活動のサポートや、Positive Parenting Program(Triple P)といった非営利法人活動の広報への資金協力を通じ、効果的な防止策の実施に寄与している。Triple Pはコミュニティ全体を対象とした広報や、多様な機会を利用したコンサルテーションに加え、実際に問題に直面している家庭に対してはより具体的な支援やカウンセリングを実施する教育および支援プログラムであり、実質的な成果を上げている。また、CDCが諮問するHealth Resources and Services Administrationの家庭訪問プログラムでは、児童の発達や養育に関する支援および養育者指導と訪問実施者の教育を行っている。家庭訪問は伝統的な保健衛生の範疇であり、費用対効果の高さも評価されている。州レベルでの児童虐待対策は未だ確立途上であるが、児童保護、刑事司法、教育などの各部局と公衆衛生の連携により、個人およびコミュニティを対象としたエビデンスに基づく児童虐待防止策の開発と発信が可能となる。こういった努力が児童を虐待から守り、発達に重要な時期における安全かつ安定的な養育関係構築の促進を可能とするであろう。
