State-Specific Trends in Lung Cancer Incidence and Smoking － United States, 1999-2008

肺癌の発症率および喫煙の州特異的傾向－アメリカ、1999～2008年

アメリカでは癌による死亡は肺癌が最も多く、そのほとんどが喫煙および二次喫煙によるものであることから、CDCは州別の肺癌発症率を1999～2008年のNational Program of Cancer Registries(NPCR)およびNational Cancer Institute’s Surveillance, Epidemiology, and End Results(SEER)プログラム、喫煙に関しては1994～2009年のBehavioral Risk Factor Surveillance System(BRFSS)データを用いて分析した。その結果、男性の肺癌の発症率は全国的に減少しており、APC(年間変化率)は1999～2005年:-1.4%、2005～2008年:-2.9%であり、西部での減少が速かった(1999～2006年:-2.3%、2006～2008年:-3.9%)。女性における肺癌発症率は全国的には1999～2006年に増加し(APC=0.5%)、その後、減少に転じている(APC 2006～2008年:-2.2)。州別では調査対象44州のうち、男性は減少:35、不変:9、女性は減少:6、不変:24、増加:14州であった。肺癌の発症率は過去5年間の喫煙率および禁煙率と有意に相関し、男性ではそれぞれ0.72(p＜0.001)、-0.55(p＜0.001)、女性では0.69(p＜0.0001)、-0.33(p＜0.001)であった。以上、肺癌の発症率の低下と喫煙率の低下に相関性が認められ、今後も禁煙および受動喫煙の阻止の推進が重要であることが示唆された。
