National and State Vaccination Coverage Among Children Aged 19-35 Months － United States, 2010

19～35ヶ月齢の小児における全国および州別ワクチン接種率－アメリカ、2010年

National Immunization Survey(NIS)はRDD(Random-digit-dialed)方式により小児予防接種プログラムのモニタリングを行っており、今回、2007年1月～2009年7月に出生した19～35ヶ月齢の小児を対象とした2010年のワクチン接種率のデータがまとめられた。その結果、MMR(麻疹、おたふく風邪、風疹)の1回以上接種率は2008年:92.1%から2009年:90.0%に低下したが、2010年には91.5%に増加、Hibワクチンの追加接種を含む完全接種率は2009年:54.8%から2010年:66.8%に増加した。また、最も新しく接種が推奨されたロタウイルスワクチンの接種率は43.9%から59.2%に増加していた。A型肝炎(HepA)ワクチンの2回以上接種率は46.6%から49.7%、B型肝炎(HepB)ワクチンの生後3日以内の初回接種は60.8%から64.1%に増加、PCV(肺炎球菌結合型ワクチン)の4回以上接種率は80.4%から83.3%に増加し、7種ワクチンシリーズ(4:3:1:3:3:1:4、DTP/DT/DTaP:4回以上、ポリオ:3回以上、麻疹混合ワクチン:1回以上、Hib:3回以上、HepB:3回以上、水痘:1回以上、PCV:4回以上)の接種率は63.6%から70.2%、Hibを除いた場合は70.5%から72.7%に増加していた。目標とする接種率(90%以上)を満たすワクチンはポリオ(93.3%)、MMR(91.5%)、HepB(3回以上:91.8%)、水痘(90.4%)であった。人種別の接種率は白人にて最も高く、また、州による接種率にも相違が認められ、ロタウイルスワクチン接種率はメイン州(42.1%)～デラウェア州(82.1%)、HepAワクチン2回以上接種率はメイン州(27.2%)～ジョージア州(64.4%)、生後3日以内のHepB初回接種率はバーモント州(21.4%)～ケンタッキー州(83.3%)などに大きな差を認め、Hibを除く7種シリーズの接種率はネバダ州(61.3%)～ウイスコンシン州(82.7%)であった。今後、接種率の低い集団の特徴や州による対策の相違などを分析し、接種率の向上を推進する必要性が示唆された。
