拡大HIV検査イニシアティブの結果－25管轄地域、アメリカ、2007～2010年

アメリカでは2008年末の時点で生存HIV感染例120万人のうち約20%が感染を認識していないと推定されている。HIV感染の検査、診断、治療、高活性抗レトロウイルス療法(HAART)および感染直後の予防対策により罹患率、死亡率および伝播リスクが抑制されることから、CDCは2006年、13～64歳に対しHIVスクリーニング検査を受けるよう勧告を出し、2007年には国内25の保健省が管轄する地域において拡大HIV検査イニシアティブ(ETI)を開始し、とくに非ラテン系黒人に対するHIVスクリーニングおよび診断を強化している。ここではその結果を報告する。2007年10月～2010年9月におけるHIV検査数は2,786,739であり、うち29,503(1.1%)が陽性、18,432例(0.7%)が新規診断例であった。このうち追跡可能であった17,247例では、15,737例が検査結果を受け取り、12,711例が医療機関を受診、14,234例がパートナーサービスを受けていた。性別では男性が全検査数の55%、新規診断例の72%を占め、陽性率は男性(0.9%)が女性(0.4%)の約2倍高値を示した。人種別では非ラテン系黒人が非ラテン系白人およびラテン系（それぞれ60%、18%%、16%）に比べ検査数は約3倍、新規診断例(70%、14%、12%)は約5倍多く、陽性率はそれぞれ0.8%、0.5%、0.5%と非ラテン系黒人が約1.6倍高値を示した。ETIの開始からの3年間で検査を行う施設は1,331ヶ所となり、性感染症(STD)クリニック(281、21.1%)、地域の保健センター(270、20.3%)が多く、新規診断例はそれぞれ3,302例(19.7%)、2,061例(12.3%)認められた。また、救急外来108ヶ所(8.1%)にて5,048例(32.3%)の新規感染例が認められている。
