CDCの最新研究:クラミジアの予防:疾病負荷および続発症抑制のための挑戦と戦略

クラミジアはChlamydia trachomatisによる性感染症であり、2009年には1,244,180例が報告されている。思春期齢および若年成人における発症が多く、14～24歳での発症率は25～39歳の約3倍高いと報告されている。クラミジアは無症候性であることが多く、症状が発現した場合、男性では尿道炎、女性では子宮頸管炎として現れる。その後、生殖器官を上行し、男性では精巣上体炎、女性では骨盤炎症性疾患(PID)を来たし、PIDは卵管機能の消失など卵管性不妊症や子宮外妊娠、慢性骨盤痛などに進行する。クラミジア感染予防は卵管性不妊症の予防にもなることから、1993年よりスクリーニング検査が推奨されており、現在、CDCとU.S. Preventive Services Task Force(USPSTF)は性交渉をもつ25歳未満の女性および新しいあるいは複数の性行為パートナーをもつ感染リスクの高い25歳以上の女性に対し、年1回の検査を推奨している。また、避妊具の使用などはHIV感染症などクラミジア以外の性行為感染症の予防にもなる。さらに感染が確認された場合にそのパートナーに対する処方を同時に行う迅速パートナー療法(EPT)やパートナーと一緒に受診するBYOPなどにより再発の予防が可能となり、感染サイクルの遮断に有効である。
