Progress in Immunization Information Systems － United States, 2009 　

予防接種情報システム(IIS)は予防接種データを収集・統合した信頼性の高い母集団ベースコンピュータ情報システムであり、アメリカの6歳未満の小児の IIS登録率を95%に増加させることはHealthy People 2020の目標のひとつとなっている。この目標に向けた進歩を評価するため、CDCはIIS参加56地域(50州、5都市、ワシントンDC)のうち53地域の2009年Immunization Information Systems Annual Report(IISAR)の調査データを分析した。また51地域を対象に、IISデータの品質基準のうち3種類(National Vaccine Advisory Committee[NVAC]が推奨する17種類の中心データの蓄積、ワクチン接種後1ヶ月以内の情報受信・処理、Health Level Seven[HL7]標準による情報交換)の遵守状況を検討した。6歳未満の小児のIIS登録率は、2008年の75%(1,770万人)から2009年には77%(1,840万人)に増加した。2009年には23地域(43%)で6歳未満の小児のIIS登録率が95%となり、10地域(19%)は 80～94%であった。2009年のIISデータの品質基準遵守状況については、6歳未満の小児の97%以上でNVAC推奨中心データ6種類(姓、名、出生日、性別、ワクチンの種類、接種日)が完全に記録されており(記録率は2008年と同等)、残りの11種類のデータ中9種類の完全記録率は2008年よりも増加した。またIISデータの70%はワクチン接種後1ヶ月以内に受信・処理されていた(2008年:67%)。さらに30地域(59%)はHL7標準メッセージの送受信が可能であり、4地域(8%)は送信または受信が可能であった。37地域(73%)では、一部の予防接種データが電子化健康記録 (Electronic health record[EHR])システムから直接IISに提供されていた。IISとEHRシステムの相互運用の促進によって情報交換の一貫性が増し、インターフェイスの費用が減少すると考える。またIISデータの正確性、適時性、完全性の向上によりIISに基づいた接種率評価の質を改善することができる。
