世界的な麻疹コントロールの進歩と死亡率の低下－2000～2007年

1963 年以降、麻疹に対して有効なワクチンが使用されるようになった後も、途上国では年間750,000名の小児が麻疹のため死亡しており(2000年データ)、WHOは2010年の麻疹による死亡数を2000年に比べ90%低下させることを目標としている。麻疹含有ワクチン(MCV1)の接種率は2000 年以降漸増しており、2007年には82%に達しているが、地域差は大きい。2007年、1歳までに初回接種を受けなかった小児は2,330万人と推定されており、インド(850万人)、ナイジェリア(200万人)、中国(100万人)、エチオピア(100万人)と未接種例は途上国に多く認められた。サーベイランスに関しては、WHO加盟国193ヶ国中162ヶ国(84%)が参加しており、2007年に麻疹症例を認めた国は178ヶ国(92%)、発症数は全世界で2000年:852,973例から2007年:279,006例と大きく減少し、2000～2007年における麻疹による死亡者は2000 年:750,000例から2007年:197,000例へ74%減少していた。このうち5歳未満の小児は679,000例から177,000例に減少、減少幅は東地中海諸国にて90%、アフリカにて89%と大きく、この期間中に全世界において麻疹コントロールにより約1,100万例の死亡を回避し、うち予防接種率の増加やSIAsの実施などの促進的介入により約360万(33%)の死亡を回避したと推定された。
