Preventable Measles Among U.S. Residents, 2001-2004

アメリカ住民における予防可能な麻疹,2001～2004年

アメリカでは風土病的麻疹は根絶されたが、その疾患がまだ風土病となっている地域から輸入され続けている。アメリカ住民において麻疹は海外に居住あるいは渡航中に感染し帰国した者や感染した旅行者との接触などを介して伝播する。この報告は、2001-2004年に州および地域の公衆衛生局からCDCに報告されたアメリカ住民の麻疹確定例に関するサーベイランスデータをまとめたものである。麻疹確定例のうち、Advisory Committee on Immunization Practices(ACIP)により麻疹含有ワクチン(MCV)接種が勧告されていたが1回も接種しなかった患者を予防可能例とした。一方1回以上MCVを接種した者、ワクチンを接種せずワクチン接種も勧告されていなかった者、1957年以前に生まれた者(小児期に自然疾患により免疫されたと推定)は、予防不可能例と考えた。2001-2004年、CDCには麻疹患者が251例報告され、そのうち177例(71%)はアメリカ住民、74例(29%)は非在住者であった。アメリカ住民177例中100例(56%)は予防可能例、77例(44%)は予防不可能例であった。予防可能100例中43例(43%)は輸入感染例(国外旅行者)、57例(57%)は国内感染例であり、輸入感染例のうち17例は生後6-15ヶ月の乳児であった(6-11ヶ月:12例、12-15ヶ月:5例)。予防不可能77例のうち国外旅行者は12例(16%)であり、このうち11例はMCV接種を1回以上受け、1例は1957年以前に生まれていた。また非旅行者65例(84%)では、生後15ヶ月以下の乳児29例(ルーチンでのMCV接種時期は生後15ヶ月ごろ)と1957年以前に生まれた6例以外全例がワクチン接種を受けていた。この期間中には4回のアウトブレイクが発生した。アメリカ住民における麻疹感染を予防するためには、現在のACIPワクチン接種勧告に従い海外旅行者、特に生後6-11ヶ月の乳児に対するワクチン接種(生後12ヶ月以上の場合には2回接種)を確実に行う必要がある。
