Advisory Committee on Immunization Practices Recommended Immunization Schedule for Adults Aged 19 Years or Older — United States, 2016

Advisory Committee on Immunization Practicesの推奨する19歳以上の予防接種スケジュール ― アメリカ、2016年

2015年10月、Advisory Committee on Immunization Practicesは2016年の19歳以上の成人に対する予防接種スケジュールを承認した。2015年のスケジュールからの変更点は、1)13価肺炎球菌結合型ワクチン(PCV13)接種後の23価肺炎球菌多糖体ワクチン(PPSV23)の間隔を免疫不全症、解剖学的/機能的無脾症、脳脊髄液漏出、人工内耳のいずれも該当しない65歳以上の場合、6～12カ月から少なくとも1年間へ変更し、これらのいずれかに該当する19歳以上では少なくとも8週間とする、2)血清型B髄膜炎菌(MenB)ワクチンは10歳以上でMenBによる疾患のリスクを認める場合に接種する、3)MenBワクチンは思春期齢または若年成人(16～23歳)に接種する、4)9価ヒトパピローマウイルス(HPV)ワクチン(9vHPV)の接種を女性、男性ともに定期的に行う、が挙げられる。接種スケジュール表では、髄膜炎菌が4価結合型(MenACWY)または多糖体(MPSV4)に表示が変更され、MenBが追加された。医学的およびその他の徴候に基づき推奨される接種については、PPSV23が1回または2回接種から1、2または3回接種に変更され、免疫不全状態または機能的無脾症の場合は3回接種が可能となり、Hibワクチンは1または3回接種から3回接種(血液幹細胞移植レシピエントのみ)となった。脚注ではインフルエンザ、肺炎球菌、髄膜炎菌およびHPVワクチン接種について、1)18歳以上の卵アレルギー患者には卵タンパク質を含まないリコンビナントインフルエンザワクチンを接種する、2)肺炎球菌ワクチンについてはPCV13とPPSV23との接種間隔を少なくとも8週間以上とする前出の条件対象を19～64歳の成人から19歳以上の成人に変更し、介護施設または長期療養施設に入居する19～64歳の成人をPPSV23の接種推奨対象から外し、年齢および医学的条件から適切な接種を行うこととする、3)MenBワクチンは10歳以上で接種条件を満たす場合とし、16～23歳に対しMenB-4Cの2回接種またはMenB-FHbpの3回接種を血清型B髄膜炎菌性疾患の短期予防のため接種する、4)9vHPVワクチンの接種対象に女性および男性若年成人を追加し、女性には2vHPV、4vHPV、9vHPV、男性には4vHPV、9vHPVが使用される、となっている。
