麻疹根絶に向けての進歩 ― 東南アジア地域、2003～2013年

2013年に、WHO東南アジア地域の第66回地域委員会は綿密な事前協議後に、2020年までの麻疹の根絶および先天性風疹症候群の制御の目標を採択した。推奨される戦略には、1)定期接種または追加予防接種活動(SIA)による麻疹および風疹含有ワクチン2回接種の接種率95％以上の達成および維持、2)推奨される評価指標に合致する高感度およびタイムリーな症例に基づいた麻疹サーベイランスシステムの開発および持続、3)正式に認可された麻疹検査ネットワークの構築および維持、4)麻疹アウトブレイクのタイムリーな同定、調査、対応の耐性が挙げられる。東南アジア地域では、2003年以前に麻疹含有ワクチン(MCV)の初回投与(MCV1)は11加盟国すべてで導入され、2003～2013年にMCVの2回目の投与(MCV2)が導入された国は2カ国から9カ国へ増加した。2003年から2013年までに、MCV1およびMCV2の接種率は67％から78％および6％から53％へそれぞれ上昇した。接種率95％以上を達成したのは、MCV1で4カ国、MCV2で3カ国であった。2003～2013年に、麻疹SIAはタイを除くすべての国で実施され、推定で2億8千6百万人の小児が予防接種を受けた。2013年までに、地域のすべての加盟国で疑い例の検査での確認を伴った麻疹サーベイランスが実施され、また、地域内の研究室ネットワークは拡大し、熟練した34の研究室およびタイの参照研究所が含まれている。2003～2013年に、全体で麻疹アウトブレイクが疑われた5,680件のうち、5,166件(91％)が綿密に調査され、これらのうち検査で確診されたアウトブレイクは、麻疹49％、風疹28％、麻疹と風疹の混合が10％であった。2003～2013年に、地域での年間の麻疹の発生率は73％減少し、人口100万人あたりで59例から16例へ減少した。地域内の5カ国では、2013年の人口100万人あたりの麻疹発生率が5例未満で、ブータン、北朝鮮、モルジブでは症例の報告は0例であった。2013年では、地域全体の研究室で確認された麻疹アウトブレイクが248件、研究室で確認された麻疹と風疹の混合アウトブレイクが14件報告され、全体の麻疹症例は10,108例であった。症例の年齢分布は、1～4歳(35％)が最も多く、次いで5～9歳(30％)であった。2003～2013年に地域内の症例から分離された麻疹ウイルスの遺伝子型は、インドでD4、D7、D8、インドネシアでD8、D9、G2、モルジブでD5、ミャンマーでD5、D9、ネパールでD4、D8、スリランカでD8、タイでD5、D8、D9、G2であった。地域内の麻疹根絶の達成のために、MCVの2回接種率が95％未満の各国のさらなる努力、定期予防接種の強化、定期的および高品質なSIAの実施、麻疹の症例に基づいたサーベイランスおよび検査での診断の強化が必要である。
